Options - Physicians
Affordable health care choices are available through two plans that offer comprehensive
coverage: Health Options (HMO) and Blue Cross Blue Shield (PPO). A pharmacy
program is also available to meet prescription needs. Employees are eligible
after 90 days of employment.

Comprehensive dental coverage is offered through three plans: Aetna Dental
(PPO) and (DMO). Employees are eligible after 90 days of employment.
Vision
Plan
Employees receive discounts
on vision care services and goods purchased. Employees are eligible after 90

Life
Insurance
Employees are provided with
basic Group Term Life Insurance at no cost together with their Health Insurance
Benefit. Accidental Death & Dismemberment Insurance, supplemental and dependent
coverage is available at competitive rates. Employees are eligible after 90
days of employment.
Retirement
Plans
BCBSF helps employees prepare
for the future through two retirement programs: the Pension Plan and the Salary
Deferral 401(k) Retirement Program.
401(k)
Information
Employees are eligible
after one year of continuous service and attainment of age 21. Open
enrollment is in January or July of the following year. The Program allows
you to save - on a pre-tax basis - between 1-20% of your salary (in concert
with IRS guide-lines). The company matches dollar per dollar on first 4%. Employees
have immediate and 100% vesting on contributions, company match and investment
earnings. Pension Services, Inc. (305) 595-5500 currently manages 401(k) funds.
Profit
Sharing Plan
Employees participate in a Pension Plan funded by the company. Employees are
eligible after one year of continuous service and attainment of age 21. You
vest, or gain a right to this benefit, after completing five years of service.
Manulife Financial currently manages Profit Sharing funds.
Short
Term Disability
The insurance pays a portion
of the employee's salary for a maximum of six weeks, after a five-day waiting
period. Employees are eligible after 90 days of employment.
Long
Term Disability
In the event of an extended illness (after six months of continuous disability),
the insurance pays a portion of the employee's salary. Employees are eligible
after 90 days of employment.
Flexible
Spending Accounts
Flexible spending accounts
allow employees - on a voluntary basis - to set aside a portion of their pre-tax
income to offset the cost of approved health care or dependent care expenses.
Employees are eligible after 90 days of employment.
Paid
Time Off
The company pays for time
away from scheduled work-time (e.g., vacations, illness, family illness, etc.).
Full-time and part-time employees begin accruing time off as of the first day
of employment. Employees are eligible after 90 days of employment.
Corporate
Holidays
The company pays for 7 corporate
holidays. Employees are eligible after 90 days of employment.
Leaves
of Absence
The company supports approved
leaves of absence, including jury duty, military leave, Family and Medical
Leave (FMLA) and management-approved general unpaid leave.
